October 23, 2024- Across the U.S., there have still been alarming cases of preventable harm within healthcare settings despite efforts to improve patient safety. According to ECRI, nearly all the safety hazards in healthcare today can be addressed by eliminating four widespread weaknesses that fuel preventable harm in the delivery of care, including neglecting system-based solutions, overlooking human factors engineering, stifling “Just Culture” in the workforce, and ignoring health inequities.
