June 1, 2026- Geisinger has received new funding to expand and enhance hypertension care across its health system, a move aimed at improving blood pressure control for thousands of patients through more accessible, technology-enabled treatment.
The funding, provided by the Patient-Centered Outcomes Research Institute (PCORI), will support expanded use of remote patient monitoring and self-measured blood pressure programs across 94 Geisinger clinics. The initiative is designed to help as many as 10,000 patients better manage hypertension, one of the leading risk factors for cardiovascular disease, stroke, kidney disease, and other serious conditions.
As part of the program, patients with uncontrolled or complex hypertension will receive additional support through a team-based care model that includes pharmacists and advanced care providers. Clinicians will be able to more closely track patients’ blood pressure readings remotely and intervene earlier when treatment adjustments are needed.
“This funding allows us to use proven approaches like remote patient monitoring across our system, helping us reach more patients and intervene earlier,” said Alexander Chang, MD, a nephrologist and associate professor of clinical research at Geisinger.
Geisinger leaders say the initiative reflects a broader effort to strengthen chronic disease management by combining digital health tools with coordinated care teams. The program also emphasizes patient engagement, encouraging individuals to actively participate in monitoring and managing their blood pressure at home.
